Douglas wants everyone to say Spiro has the better physique. He wants to be the "victim of injustice," because it gives him a small distraction from the facts, which all point to his degeneracy.
He blinds himself to the reasons everyone treats him "unfairly", which are actually legitimate: for example, his tiresome self-praise and pointless pursuit of an "aesthetic physique" in the face of his hideous bad looks.
This behavior is typical of "bodybuilding enthusiasts", who pursue muscle blindly and let their life fall into ruin. Whenever criticised, they fall back on their "physical superiority", which only appears as such to them because they are, by nature, incapable of placing their physique in perspective, and refuse to see, for example, that they focus on muscles purely as an excuse to avoid competing where they cannot, e.g. in business, athletics, etc.
