i've never seen it before, both left and right side of the lower back are affected, right side having more prominent (inflamed) protruding to the exterior

i had never thought of lower back as an area to pin, but i can't think of it being anything else atm

If you pin in the wrong place down there you would know about it pretty quickly.

Cysts in the muscles, but from what I dont have a clue.

First guess is subcutaneous injection
of "whatever" for local fat loss or diuresis. Lower back would be the obvious spot close to a show for attempting such.

I think it’s his nodes. Does anyone really shoot anything in that area? It’s bilateral too. His lymphatic system is probably fighting something off.

That's where many hold "stubborn fat". Not a new idea, there's been some pharma as well as UG "spot reducers" for a long time as you know. If they work is questionable.
Anyway, that's the first thing that came to mind, no idea if that's actually the case here. But I mean now that he's lean testing a new local diuretic or lipolytic would make sense.
