I think he has a gift and a curse, he's an advanced responder to slin and hgh but that's also the reason he's had these adverse changes as well. Not many other pros have had this much change in their faces. Rhoden, dexter, ramy, roelly etc. don't look too bad, even kai greene.

The change between the first and third pics is striking. Heath is starting to look like a big version of Joe Sixpack with the belly beginning to change. The way he arches his stomach out, not in, is telling. Like he can't control it any longer.

his face changed between late 20s to mid 30s due to massive acromegaly. the guys you posted just got old and flabby/grey, and a couple abused hard drugs. whereas heath's facial structure and head size changed plus his nose got bigger.

what i think the phil pics prove is that your physique weaknesses will always be weaknesses no matter how much you prioritize them

phil made the best of his weaknesses, but his chest and shoulders are still narrow even after making improvements and his arms were always oversized

You're right in pointing out there are many reasons our facial features change. Growing old is the most common one.  As the cartilage in our ears and nose breaks down, gravity lengthens them. Heath's facial changes (acromegaly ?) could be from using GH, which seems the most likely reason.
