Some sarcastic humor there. The guy he talking about needs medical help. He would do him self a real favor if the got off all the damn drugs and walked on a treadmill 30 minutes a day if a doctor okays it. Just quit lifting too for now.

Can't see the picture, but from recollection of his appearance he likely has liver damage (cirrhosis, early stages) and has ascites.

Likely other stuff as well but that is why his gut looks like that. Probably portal hypertension as well.
