Clavicle width is the issue here. I think the shape of the pecs also plays a role. Heath just has high narrow insertions for his pecs - they're never going to look wide and deep like Ronnie's or Jay's did - just the way it is. He is a bit small, structurally though. Matazarro was a wide bodybuilder - he would have had broad shoulders without lifting. Phil would look a lot narrower without his muscle mass.

I have never known bodybuilders to lie about their heights. Weights, yes.

Phil Heath is at or just under 5'9".

Kevin Levrone is 5'9.5".
