Yeah, this is why I hate getting two people to spot me. It's bad enough having one shit spotter. It's worse when the spotters really need to do their job well - they absolutely have to be in sync.
The left spotter correctly engages when its clear the lift has failed. The right spotter waits, thinking that he might be able to relift it after going down, which means the bar gets lifted up by the right spotter and dumped onto Larry as an uneven load.
He was fine until that. At that point he has to go into survival, dump the bar behind him mode, but they are now holding the bar from being dumped out back.
It's just bad spotting, period.
I don't know why so many high level squatters insist on not having safety bars (I know guys at my gym who deliberately squat using the pegs on the outside of the power rack for absolutely no good reason).
Same sort of shit has happened to me when two people have spotted me on bench. It isn't pleasant. It's a clusterfuck.
