I've got personal experience of the UK NHS system in terms of Pain Management departments/guidelines and the like. We're talking since last year, here. You're right - anything above tramadol and pregabalin 300mg tablets is out of the question unless you have a specialist pain consultant that has sorted out your 10mg,20mg or 50mg morphine tablets: that's for the hardcore diseases like cancers unless you go private.

Shouldn't really say this, but Tramadol SR makes Taffin very happy - I can totally see why people might get addicted - feels different to the other opiates/opioids to me for some reason

Hahaha!  Yes, of course - I guess what I was trying (and failing) to express was that it doesn't matter how much shit you get prescribed it's a flat rate, unlike the US system.  For example, I dread to think how much my daughter's type 1 BG meter, insulin pump, insulin, etc. would cost someone paying per item


4- lastly, I believe this is the last one, she suggested I go on ketamine. Like all day ketamine. Ketamine is what you give a soldier that you direct just dragged into the medevac who just got both his legs blown off is a disassociative. Disassociates you from reality so you don't know you're in pain you don't know where you are. When you're on ketamine you're falling down an elevator shaft backwards non-stop for a good 8 hours and you're completely paralyzed unable to move Your mind is awake but your body is completely numb and disconnected from your brain. That was her idea

2 ketamine horror stories real quick - I once snorted a shit load of what I thought was coke, turned out to be K, when it hit me time slowed riiiiiiggghhhtttt dddooowwwwnnn..... it felt like I'd accessed the same part of my brain that once slowed things down during a motor bike accident.  Not unenjoyable, just a bit unexpected

The other was when a friend took some with a huge shot of vodka - when it kicked in he turned to me and said 'I might be in trouble, I can't see properly, everything is upside down'(!)

Both situations lasted minutes and were over 25 years ago - Taffin is a good boy nowadays (despite the Tramadol comment above  ;))

Wasn't aware I have followers or follow people. Far as I was aware it was like a YouTube account. Besides some of my females clients & gfs who make videos all I got was breakdance videos over the same song some it's been deleted off my phone for What must be 6 months or more by now.

That Little bit of extra happiness, is from tramadol's effect on serotonin receptors, in addition to the "warm hug" you get from the opioid receptors

Tramadol is what started me on opiates after 14 years clean from Nubaine

I just had ACDF on c5-c6 c6-c7. Something definitely up with prescribing pain meds after surgery. I was given  nothing, worse type of pain I've ever felt. My surgeon is against opiods as he worked in the USA and said they prescribed it like candy, saw the residual effects of what they can do.

Literally have 6 screws and a plate into my c-spine holding me together and it's  no walk in the park.

Okay let me summarize what the pain team PA has suggested so far

1- because I I said it feels like it's burning and pulling off ripping off skin off my my leg she suggested Cymbalta

2- suggested pain relief for this type of wound v ivy Tylenol

3- she suggested that I switch to buprenorphine (Suboxone), which would throw me into an instant precipitated withdrawal that will last hours and hours and hours causing uncontrollable vomiting shitting pouring of sweat. All with my leg in this position

She may be a nice person she may have other skills and other areas of medicine but she is grossly unqualified for this position

I don't know what I was reading, probably Reddit, and came across posts about a new drug, Auvelity. Claims include it's very rapid acting, within a week, unlike conventional like 4 weeks. It's a combination of buprenorphine and bupropion (Wellbutrin),    buprenorpine which is similar to ketamine (disassociative). It's probably very low dose so you can function in life.

I can't recommend it, never tried it. But it does not target serotoning. I would be interested in real person reports.

I'd try it for my treatment resistant depression but it's not availablee here. I've though about psychedelics but too scared to try it. A friend said Psilocybin made him quit cigarettes for 3 months, the effect didn't last but it's interesting the reports of cold turkeying any types of substances after one session. Even if it's rare it lasts, it's still interesting. Microdosing I could do perhaps because it's sub-psychedelic doses.

Wound..?  I must have missed this - what wound?  Plastic surgery for what?  I know you've talked about your legs burning and guess the sepsis must have got into you somehow so did you have a cut or something?

(And hopefully the dilaudid boluses are kicking in by now  ;D)

Yea you can see pics on my IG..  ever removed the back straps off a deer?
They kinda did that to each side of my leg/from foot up the calve. The outside is about 15 long by 2 wide and an inch deep, the inside is "only" about 12". & I'm supposed to have a wound-vac in each side. But they want to replace the woukdvac 3x. A week....

The first time they did it is the last time I can tolerate that kinda pain, because the vac-foam suck into the wound and attaches to the tissue, and removing it is like ripping out 1" deep 2" wide strips of flesh each time... Apparently, according to the plastic surgeon because I'm not mad enough to tolerate that type of pain I've extended my recovery time to three or four months of being an inpatient either at a rehab/skilled nursing facility or UC Irvine was most likely where they do more plastic surgery.

*, So instead of the wound vac, I get daily gauze dressing changes which are still fucking painful. I'm usually like a "tough guy/suck it up and push through" you know?  I walked around /ran on a  broken tibia for 2 weeks, I was up out of the hospital bed walking after my first my hip surgery and within a couple hours, I've played through broken fibulas and collar bones and fought through broken hands and arms and a sternum..

But this pain is on another fucking level. There's laws to protect people when it comes to pain management. And pain management lady today who I thought I had a good rapport with tells me that yeah we're going to keep working to keep decreasing your your pain meds because "you're trying to recreate a past experiences from your past experiences with with opioids... I.e. fancy way of saying she's telling me I'm trying to get high.... And like I said wants to cut my doses down even though I'm probably 2 months from going home, still. & The fact they don't understand opioid receptor affinity or saturation or whatever and they still have me on methadone in the hospital despite giving me pain meds that would keep me out of withdrawal, but they don't understand the methadone is soaking up so many other fucking hope you would receptors that the Dilaudid doesn't have room to work.

 I told that bitch I haven't been high in 10 fucking years. What the fuck is my past experience have to do with the pain I'm in right now you've seen what my leg looks like open with no bandages... I shouldn't have remind you that it's not your job to tell me how much pain I should be in."

I already been contacted by three attorneys. Haven't had the energy in me to call any of them back yet, but they don't typically stick around with the smaller cases they want to go for the big fish the big insurance companies the big hospitals because they settle in exchange for a gag order so you don't talk about the poor care you received at the hospital... But I'd trade in a fucking heartbeat the '80-200,000 or whatever that'll likely get for the lawsuit to have this have never happened.
