This could have several different meanings.
He certainly looks sick in his head but also ill health.
You just get the impression that he's a truly depraved person and what he's talked about is just the tip of the ice berg. He's the type that has no moral compass, no guilt or shame to hold himself back (other than criminal law) from doing anything he can rationalize to himself.
