He claimed to be a physician, however…..
• He never completed a residency.
• He never obtained a medical license to practice in Michigan, New York, or elsewhere (confirmed via state medical board records reviewed by multiple outlets).
• He never practiced medicine independently as a licensed physician. His only direct clinical patient-care experience was a short (about four-week) sub-internship/clinical rotation at the end of medical school, which he has described himself as “cosplaying a doctor” and being “the worst doctor on the team.” 
He has repeatedly referred to himself as a “physician” (including on LinkedIn, in campaign materials, speeches, and public appearances) despite the lack of a license or independent practice. Michigan and New York have rules restricting use of the title “physician” without proper licensing, and critics (including in 2018 during his gubernatorial run and again in the 2026 Senate primary) have called this misleading.
