I remember when people used to wonder if a person had plastic surgery - "Oh you think they had work done?". There's no art to it anymore, I wonder how much they're paying for this stuff, it looks high quantity, low value.

They married rich. So their capital their whole life was their looks. When that starts to fade they can't accept it.

In a way many bodybuilders are simular. They where appreciated their whole life for how they looked. When that starts to go they also have a hard time accepting, continue PED's even though in bad health etc.
