Just a guess, but light weight working them...squats (rather any work out, but squats in particular) put your blood pressure thru the roof. If you have issues with hypertension or heart issues (which he has0, squatting heavy, even light can be a dangerous...

One has had open heart surgery and stents and the other a kidney replacement.  I won't argue that Flex had one of the best physiques I have ever seen but genetics has caught up with both of them and not in a good way.
