An insurance company sent me a statement with my name handwritten and my last name was misspelled by a single letter.

I'll do you one better. I was paying an insurance company for my car insurance and they had the WRONG VIN NUMBER on file when I called them to re-evaluate my coverage when I moved. A VIN number that didn't belong to my current car or my old car. It was close to my VIN, but off by a few numbers. Companies make these mistakes constantly.
