As does ANY extension at the elbow, it works ALL of the heads of the triceps.

If you really want impressive triceps, close grip bench more than 300 pounds.

lying extensions on the flat bench, lower the weight behind your head instead of stopping at the forehead.

Bingo. Lots of guys who don't get everything they can out of it because they lower the weight to their forehead; Levrone's one.

The rope extensions on that link also work; any extensions with the elbows raised up.

Extensions emphasize the long head much more than any other triceps exercise, leaving  them  with the best potential for size gains since the long head is the largest part of the triceps. Find a version of extension you like and you'll get soreness in the long head that you'll never get from close-grips or any other triceps exercise.

Close-grips work primiarly the smaller lateral and medial heads, and work well for some and don't do much for others. Milos isn't a big believer in close-grips for example. Bench dips, dip machine and flared-elbow pushdowns work as well or better for the same area, for some people.

As usual, squadfather and pumpster are dishing out some great training advice.  Follow that and you'll be fine.

Yeah. I was guilty of this, but now i do skulls the proper way. Can't use as much weight, but feels a lot better, whereas before all it really did was make my elbows burn.
No doubt about it...

Yes.
And to further support this, as well as the notion (also frequently suggested by Pumpster) that extensions behind the head are superior builders of the long head, read this article by a former "Mr. O.”

http://www.larryscott.com/bio/newsletter/97summer_2.cfm

I’ve been doing these as described for about a month now, and they are very forgiving on the elbows, the stretch is great, and the soreness (good PW kind) the entire length of the triceps is unequalled.

If it is inconvenient for whatever reason to use this setup, consider doing bodyweight tricep extensions (a.k.a. pushbacks, pushoffs, tiger extensions, etc.), which provide a similar feel.
Someone was just talking about those on here the other day, though I can’t find the particular thread.

I believe a thorough description appears in the Arnold Encyclopedia for those who are interested and ambitious.
