thanks everyone, including unracked got out of the clinic the dr said its infected-something to do w/ vascular its all red going down to my wrist...it feels warm and tender-when i press on it i see the white spot form and go away-feels a bit swollen too i got a script for a broad spectrum antibiotic and i will call my dr today if it gets worse i have to go to the e.r.

That's blanching that you are describing (the white spots that form when you press on your swollen skin and disappear)... this is due to extravasated blood in the skin from leaking capillaries due to localized inflammation. Sounds like broad spectrums are a good start.

MOST ER docs are morons and there is no reason to admit you, so you are wasting your time and theirs. You'll get much better care if you follow up with your doctor.
