Yes Leo is mental and the situation is manipulated so that he accepts finally accepts reality and thus prevents the need for him to receive a lobotomy. Unfortunately it fails so at the end he is about to get lobotimised.

but i'm sure at the end he knew what was coming and was infact wanting the lobotomy to finally escape the pain he was going through.. makes me chuckle when c unts on film forums all try and make out they saw the twist coming after five minutes of the film starting  c unts!!!

Exactly this. How you didn't get this when they spell it out at the end is beyond me. Now, had you asked about the ending of Inception, you could argue how it ended.

Well.. Depends on who ya ask.. One "theory" put forth by the writer was that Leo is no longer mental but knows he won't have peace ever.. So he opts for the lobotomy.. That is why he just goes to his fate knowingly and willingly.. They, the guards and doctors never say what they are there for.. Yet he goes to them willingly.. Before he does he asks "is it best to live like a monster or die like a man" .. Or something like that.. He knows what is ahead of him and does not want to live with he memories.

When I was younger I worked at the St. Louis State Asylum as a painter. Underneath the building were a bunch of tunnels that would lead to what appeared to be torture chambers. They had the classic steel hoop drilled into the wall to chain up these people. An old nurse who worked there at the time said she was in the room on many lobotomies and shock treatments. She said they would just really experiment on them doing barbaric stuff since the majority of the patients had no family come to visit. She said eventually a standards committee or something came in and shut the place down for a year due to unethical practice. She said after what she had seen in her life she felt at ease about her death, she had cancer and died a couple months later.

Doctors back in the day were free to do whatever the fuck they wanted. My grandmother was a lab rat back in the day when she had TB. damn thats crazy. That same nurse told me that she saw some doctors put maggots in the brain through a hole they drilled thinking that they would eat the diseased part. Some people thought she was full of shit but she was in her 80's and I think a volunteer when I met her. Why would she lie? Even though I was a strapping young buck 25 years ago I enjoyed hearing her stories so I always stopped by her station to chit chat.
