should he lock the elbows every time?

please.

at the lift off check the distance on the bar from his chest and then during each reps watch how far away he comes from that initial point of reference

Ah yes, I have never done 46 reps with 225, I have done 45 and I have done 500 many times, not once but at least 10 different occasions, in fact I have done 515 at least 3 separate times. Trust me Heath can do 500, you guys need to stop hating.

I think he could "anyhow" bench #505 for a single in a gym, but I would doubt it would pass at a real Powerlifiting meet. I think mid #400's for a legit paused bench with rules on foot, head, and ass position is more likely for him.
