Exactly my thought...

Then again, I am not 100% sure and always leave the door open to the possibility of there being some sort of minute potential for it to be possible.

That said, I know Derek Anthony and spoke to him not so long ago (about a month back) and he seemed to be his regular self. I didn't do a neurological evaluation, but the same Derek of old was present.

Looking back at the photos of Derek when he was younger, he's always had a tendency towards full cheeks (kind of like a chipmunk). That feature has become more pronounced as he's matured. I suspect with his recent weight loss, his cheeks are sagging a bit. If you study recent photos, you will see this is most prevalent when his head is looking down at the camera which brings gravity into the mix.

To avoid this look, he could lay on his back on the floor, thus allowing gravity to pull those cheeks back when taking photographs....or get a facelift. Surely, this is the least of his concerns right now.

I'm pretty sure his face puffyness is from his kidneys not working... shoving his body full of synthol probably isn't helping in keeping his blood clean
