Need some advice on dieting for the very obese

I have agreed to put a diet and eventually a training plan for a neighbour of mine who is very obese.

This is something i've done a ton of times for people of different sizes, but I wanted a bit of advice for this guy as he's pretty large.

58 years old, 5'11", 350 pounds - very out of shape and has to stop and catch breath even after walking 100m.

I've got his calorie intake and how we are going to approach exercise all sorted however, I just wanted a few tips in terms of the choices of foods to start him on.

'Eating what he wants, as long as he's not going over X calories per day' isn't going to work for this guy - he has a lot of eating behaviours and triggers that would fuck that up very quickly.

On the other hand, I can't start the guy too 'clean' as I think he would struggle with a drastic change - he eats very rich foods at present and loves cooking/spending money on nice ingredients.

Any help appreciated bros

I'm no expert but personally I would start with 500 less calories per day, clean up rest of diet a bit but allow the tub a bit of small leeway and build up to an hour a day of walking then go from there.
Put great big obese fuckers on an overly strict diet and they will explode and eat everything in sight, at that size small changes will still make a drastic difference

Because 95% of people on this board am aware that I know how to construct a diet you stupid little cretin.

I am simply asking for a few off the bat tips on how the more knowledgeable posters here would approach this situation.

The mediterranean diet may be best for someone who wants good fresh ingredients.

That's more or less my starting point (deficit-wise) as this is going to be a 6 month to a year thing, if he sticks to it. This is not a '12 week transformation' etc as he just has to much fat to shift.

I've gotten a 3 day food diary from him, and worked my figures from there and the usual BMR calculations etc. There are some quick fixes I can make (i.e. he was consuming over a quarter of his daily kcals in sugar based drinks - fruit juice/schloer/soda) but i'm looking for advice on how clean to start off with the guy.

I can construct a diet for a 'bodybuilder' down to the last almond lol, but this is new territory for me as I really feel for the guy and know how depressed he'll be if we start this and he fucks it up.

You need to lower the dose mate, take a chill pill  ;D you know im right and it hurt you, i understand that but you dont have to let your anger and jealousy get the better of you. Im astounded you of all people have responded this way.

I am leaning that way, in terms of including a lot of fish in the meals and keeping the flavours lively (as he loves his food/eats out a lot also).

A few things he mentioned to me when I asked about preferences/foods to avoid etc.

1. He hates cottage cheese & avacados - shame as very good foods in this type of diet.
2. He doesn't like bland foods (it shows hahaha)
3. His downfall is heavy carb-based foods, pasta dishes, lots of breads, hot puddings, and high calorie sauces with meals.

More calorie sparse foods, less of the butterfat and starchy stuff, walking only.  At that level of unfitness it's really just eat better and move around more imo.  Honestly, if he can't walk 100m he should 'consult a physician before engaging in an exercise regimen,' as the legalese always reads.  He might need to monitor his heart rate or he'll keel over.  (Dibs on his workshop tools.)

So ya.  It's no mystery how he got to 350 lbs.  He doesn't need a tailored diet and exercise plan and he knows it.  He should walk around outdoors and stop stuffing his face with high calorie foods.  To stand there and act like he doesn't know how eat healthier and get some exercise!  The balls on this fat bastard!  

But good on ya for helping him out anyway.  Maybe he'll really carry through.  And if he doesn't you can just get him a POWERLIFTER t-shirt.

IMO, eliminating or are least dramatically reducing starchy carbs: rice, pasta, potatoes, and especially bread, will give you the most weight loss bang for your buck.

If that doesn't work out there's always gastric bypass surgery. Has a good sucess rate among morbidy obese..Not without risks but the long-term mortality rate of gastric bypass patients has been shown to be reduced by up to 40%.

Diet? The guy needs therapy. That's a mental illness, not a spill over.

Just tell him to stop stuffing his fat fucking face all day long.

He doesn't need to go from eating constantly to starvation - he'll lose a ton of weight eating what for most would be a generous diet but what for him would be very little.

He is almost certainly lying to you about what he eats.

You need to focus on the frequency of his eating and what triggers it.

Like I say - this is therapy territory. The diet will fail. The man has an addiction.

Thanks for the input lads.

I'm definitely giving him a 'bread ban' as just that plus the sugary drinks was amounting to over 1000kcals daily.

I'm considering just giving him the calorie deficit to start with, and banning the above. See how that works for the first 2-3 weeks. If he shows he's serious and gets through the first month, then i'll slowly clean up the diet.

His food diary I believe was honest, as he was writing stuff like 10:15pm - 2 glasses of Pinot Grigio and 8 chocolate hobnobs lol, if he was shitting me he would have said 'couple of bisuits and a glass of wine'

Lol another gem was the next day (after 3500 plus kcals of shite) '9pm - whole bag of maynards wine gums' hahaha  ;D

In all seriousness, I believe that morbidly obese people need psych help.  Idk if eating is their sole source of enjoyment in a sad life or what but something drives them to do this damage to themselves and they need to address it or no diet will ever stick.  Maybe he just wants some moral support, someone to praise him for losing a few pounds, something to make him feel good other than eating, etc.

Fuck it's making me depressed thinking about these people.  I better go rip out a few chinups.

bro you might want to start him on a liquid diet for awhile and then maybe go vegan for awhile. I know Mike Tyson was almost 400 lbs at his worst and he went on a liquid diet for awhile and then started eating vegan and he lost about 100 lbs.
