I think his being carelessly pinned is what led to his downfall. Looks more like the hivvy, than the covvy.

Looks like the result of not lifting or eating right x 6 weeks coupled with no exercising or using steroids.

Most guys do not have a good base to begin with before they go hard on the steroids. As a result of that, once they stop lifting, injecting and severely reduce their meal intake, they turn into that (as seen above).

Also, echoing what someone else said, this guy looks like a classic HIV'er without the steroid cocktail that many of them use. Off the steroids, a lot of those young gay guys that were tiny to begin with start to resemble vampires (Nosferatu) or former zombies.
