I don't know what an HMO is exactly, looked it up but I don't think I had anything like that.
Actually all the follow ups were with surgeons, not the surgeon. I met the actual surgeon for 2 minutes after I woke up. One time another surgeon was supposed to look at my incisions/scar but just lifted the dressing up half an inch, didn't look, and then just said "rehab" and disappeared from the room, took less than 30 seconds lol. The rehab folks were just clueless about anything and couldn't answer any questions, just had the xorexed generic exercise descriptions. I was also prescribed rehab for my upper back, so they show me a lat pull with 20lbs of load and then face pulls with these flimsy rubber band slips... then they ask me why I stopped coming, if I'm not serious about getting better 
Most rehabs are designed around getting the normal person back to being normal.
Not for the kind of shit everyone here does.
You can fall into a similar situation here if you're not educated on the subject like Krank. I have had experiences with both.
For example, had a situation like yours for a slap labrum tear, where they wanted me to come in each week to band work I could have easily done at home (and did).
But, years ago when I went into PT for when I had a clavicle excission/subacromial decompression in addition to having a hip flexor/IT tear rehabbed at the same time, the rehab guy said to me "you look like you train, so do you want to do this the normal way, or fucking go after it and get it over". I took the latter, and he preceded to stick his thumb into my shoulder and started grinding away scar tissue. Needless to say his way was pretty effective, but hurt like a bitch.
