As crazy as Lenny seems to be sometimes, I just don’t get how he can go along with his everyday life, seemingly happy with his bodybuilding career, if he’s on the verge of liver failure. He either doesn’t care about living or just doesn’t think it’s anything very serious.

He is fully delusional, possibly a delusional disorder and severe body dysmorphia. I watched a video and he was talking about how not cleaning up preserves calories to build muscle, he is out to lunch.

He has to know it's serious, he appears to have jaundice in some videos. He must feel like melted shit.

He will live forever though, then you have fit guys just drop dead while people like Lenny torture themselves and nothing happens.

I agree.  I trained at the same gym as Lenny and Genova in 2006-2008 and they're both delusional.  I remember Lenny training with his friend Andrew who is legit strong and big.  Lenny hit a set of bench and then hit a most muscular in the mirror talking about hows he's going to step on  stage at 300 lbs riped to the bone.  I remember Genova catching me in the locker room and telling me to "check out his back" because he thought it was looking good.  He didn't have a drop of muscle on his back.  Still doesn't.
