I thought ascites was part of end stage cirrhosis...it seems like he would be laying in bed sick all day. How does he have the energy to workout everyday and ride his bike all over town?

Not at all, I have seen many with ascites due to cirrhosis and they can still function quite fine at times. They have good and bad days, I can only imagine the amount of drugs he is on.

It is ascites, why i don't know, I suspect liver fibrosis but who knows as there a few other causes. How he functions the way he does I do not know, the patients I have seen can hike etc but require puffers amongst other things to do so.

Unrelated I would suggest, I think that is from possible organ growth, excess visceral fat from elevated insulin levels and abdominal distention from overstretch due to not maintaing core strength when lifting with some overeating mixed in.

With ascites you tend to be unable to flex the muscles and it is very veiny (due to portal hypertension) with a almost shiny appearance of the skin.

Lenny clearly must not care about living.

As crazy as Lenny seems to be sometimes, I just don’t get how he can go along with his everyday life, seemingly happy with his bodybuilding career, if he’s on the verge of liver failure. He either doesn’t care about living or just doesn’t think it’s anything very serious.

In all seriousness, Lenny would make a great live commentator for bodybuilding shows.  It definitely wouldn't be boring.
