  So make sure to strengthen your rotator cuff. The best way to do this is with isometric lateral raises. Grab heavy dumbbells and perform lateral raises, but hold the weight for 10 seconds when your arms are at a 90 degree angle from the floor. Then increase to 15 seconds and so on until you can hold it for 30 seconds. After that, increase the weight and rpeat the preocess so on and so forth.

  The lungs work to suck air in by creating a difference in pressure due to vacuum. If you perforate one of your lungs, a vacuum is no longer present and the lungs cannot work. This is obvious. Without Oxygen, you die in minutes. So you are right that you won't die instantly, but for all practical effects it is the same.

Your lung do not create the lower pressure you are describing.  Also, the injury you describe can take minutes or hours to create any significant morbidity.  Mortality can ensue, but it is far from instant or certain.  So no, they are not the same.

If left for a prolonged period of time, yes you can die from what is known as a tension pneumothorax (very rarely does this ever occur).  For the most part and in most situations, what you will develop is a pneumothorax (an accumulation of air between your lung and chest wall) on the side of the affected lung.  While dealing with a pneumothorax is no fun for anyone, you can certainly breathe while it's present.  What they do for such things depends mainly on the actual size of the pneumothorax, some cases are just kept in observation and resolve, while other cases require the insertion/placement a chest tube onto the affected side, which in turn restores the negative pressure needed for your lung to re-expand.

So, again, it is much more probable that a gym goer will suffer from a complete lesion to the C1-C7 cervical region of the spine, by way of a heavy squatting injury, that causes instant quadriplegia.

 Rotator cuff injury and here is why: the only thing that all torso exercises, be it back, pec, delt or arm movements have in common is that they all involve the rotator cuff joint to one degree or another. Even triceps exercises, like French presses and triceps pulldowns, require you to strain your rotator cuff joint by keeping it steady. It is impossible to perform any upper boy exercise except for barbell curls without the involvement of the rotator cuff joint and the middle deltoid head that is the primary muscle involved in moving it. If you tear your quad, the second largest muscle in the body besides the buttocks, you can still work your calves and himstrings and all your upper body. If you tear your latissimus, you can still work your mid and lower back as well as your lower body. Both the latissiums and quadriceps are much larger muscles than the delt muscles involved in moving the rotator cuff joint, and yet tearing the latter has far more devastating consequences for bodybuilding.

  If you tear your rotator cuff joint, your bodybuilding is over forever. You won't be able to perform any upper body exercise, and it never heals 100% again. Recovery takes 6 months to a year, and you will lose 50% of your upper body strength forever. You will only be able to bench, military press and barbell row 50% of the weight you once could. It is the end.

My rotators are fucked, mainly because when I was younger I went too heavy in an uneducated way - but having said that, I would prefer that to a bicep or a pec tear - a rotator might fuck with you, but it cant be seen, bicep and pec tear are painful and slow to heal, and you are truly never the same again, especially if you did it through negligent or heavy training - it puts a fear in your mind you could do without.
