I've always believed you were a Doctor.

It is NOT that difficult to become a Doctor, especially if you go the simple route (i.e. Internal Medicine) and stick to city/state schools for both undergrad and med school.  That way your debt is practically zero and your competition while getting through premed courses isn't as fierce and due to attending city/state schools, the curriculum is always watered down as compared to high-end private schools (big name schools).  Also, stick to internal medicine and you will train alongside Caribbean med school graduates and while the starting pay after residency will be around 150K (with malpractice covered), you have the prestige of being a Doctor and get the perks that come with it (license plate that says MD, cute credit card with MD after your name, prescriptive privileges etc.).

What is difficult is to become a Doctor (MD or DO) and then go on to do a surgical residency (5 extra years) and then maybe an extra specialized surgical residency like say Neurosurgery or Cardiothoracic surgery (both of which are an extra 2-3 years of training on top of the 5 year surgical residency).

When you look at what those guys go through in terms of training here in the USA, it becomes a bit ridiculous.

For a Cardiothoracic surgeon, you have to put in:

4 years of undergrad + 1 year in between to apply for med school + 4 years of Med School + 5 years of general surgery residency + 3 years of CT surgery residency = 17 years of training!!

That's pretty crazy..  Now if you go the Internal Medicine route, it would be WAY easier:

4 years undergrad + 1 year in between to apply for med school + 4 years of med school + 3 year residency = 12 years (not terrible)

So do I believe you?  You betcha!

Disgnose me.  My lung capacity is down to 75%.  It does not improve on albuterol nebulizer treatment. I have GERD but it is successfully treated with ppi.  EKG is normal.  Chest X-ray clear and heart normal size.  46 allergens tested and none detected positives. this problem began to present when I moved 6 years ago and has progressed.

OMR, you are very mistaken in your belief that Internal Medicine is easy. I am currently board certified in internal medicine and nephrology. Thus, let us look at the schooling. Four years of college. No one year off, you can apply to medical school in your fourth year of college. I got in the top 5% on my MCAT and finished top quarter in my medical school. I obtained my M.D. in four years. Internal medicine for three years. I then did a nephrology fellowship for three years. General surgery is a dying field. Half the classes are not filled now a days, as surgeons get royally screwed once they graduate. Trust me, I know all the local surgeons and the vast majority are now hospital employees. CTS or cardiothoracic surgery usually is done post general surgery and is max 2 years unless you do a research year. Now neurosurgery is impressive. Seven years residency and that does not include a fellowship. Most neurosurgeons are damn near forty when they get out  ;D.  Anybody who gets through and passes their boards has worked hard. However, as has been surmised on the board, a large number of docs are social retards and generally not overly social people. The docs who are charming and intelligent do very well.

As for calcium and muscle cells, calcium is intrinsic to active muscle contractions via actin - myosin. In a nutshell, lack of calcium or hypocalcemia causes tetany which is where the muscles cannot contract and actually twitch and spasms. Hypercalcemia does not increase muscle contraction or strength. TBH, M.D. don't deal in pathophysiology to the extent that Ph.D's do. If you really want the physiological mechanism whereby a muscle contracts speak to a doctor of physiology. They can bore you at length about voltage gated channels and sarcoplasmic reticulum  ;D
